The SDOH Manager, Program Development, leads the development, implementation and evaluation of strategies focused on improving community conditions that impact health in accordance with organizational population health and community strategic goals.

The SDOH Manager, Program Development, is responsible for the successful execution of evidence based health improvement initiatives with specific focus areas around but not limited to food security, access to healthy options, housing and other social needs as measured by engagement of individuals, program outcome measures, and other pertinent metrics within and outside of the healthcare system. This position supports broader CHNA processes and implementation strategies and internal initiatives to support individuals with identified needs including determining appropriate metrics and monitoring the progress of the implemented interventions. This position builds and maintains strong relationships with internal and external community stakeholders through active participation in community based initiatives and seeks out opportunities to shape policies, practices, and upstream interventions in health and non-health sectors in ways that promote health and health equity. The manager is responsible for gathering community input via formal and informal mechanisms, and collecting and interpreting data points to assist in making data-driven decisions around targeted interventions.

This position supports organizational five B goals by ensuring quality and customer experience standards are met, demonstrates a commitment to team work via oversight of outreach staff and related operational needs, and actively participates in performance improvement efforts. The SDOH Manager, Program Development, works closely with the Social Determinants of Health (SDOH), Project Manager.

EDUCATION: Bachelor's degree with 4-5 years' related experience required.. Bilingual preferred.
LICENSE/CERTIFICATION: Valid Driver's License required.
EXPERIENCE: 3-5 years' experience in an administrative role in project/process management. Leadership experience preferred. Capacity to coalesce diverse members of the community around a common goal. Excellent facilitation, communication and leadership skills required.

Demonstrates the technical competencies as established on the Assessment and Evaluation Tool.

Essential functions of this position are listed on the Assessment and Evaluation Tool. This position requires occasional walking and standing and may have prolonged periods of sitting. Frequently works with such equipment as computer terminal, fax machine, copier, telephone and scanner. Position requires some travel, flexibility of hours and may require lifting up to 40 lbs.

This position reports to department leadership and supervise incumbents as assigned.
