The NKF Patient Program Manager will provide day-to-day management of NKF's patient and donor programming, including NKF Peers and THE BIG ASK: THE BIG GIVE (BABG) programs. The Patient Program Manager will be bilingual in both English and Spanish.

PRINCIPAL DUTIES AND RESPONSIBILITIES

  • Provide day-to-day management support for patient and donor programs, including NKF Peers, BABG programs, and related projects. Provide ongoing training to NKF Cares staff about these programs, along with processes and procedures for call intake.
  • Track and analyze program data and metrics; develop outcomes reports.
  • Review applications from people interested in being, or receiving support from, Peer mentors and Patient Navigators.
  • Determine and facilitate next steps for each applicant, which could include referral to an NKF social worker, immediate matching for those needing support, or referral to other resources.
  • Match eligible patients, living donors, and potential donors with a Peer mentor or Patient Navigator, manage the support process, and provide follow up support, as needed.
  • Provide administrative and technical support to Navigators, mentors and those needing support throughout their involvement with the program.
  • Monitor the progress of all participants in the programs.
  • Provide superior project management support for programs, ensuring that timelines, deliverables, and quality standards are met or exceeded.
  • Demonstrate consistent quality of administration, communications and systems.
  • Perform additional duties as required, which may include involvement with other NKF programs or projects.

MINIMUM QUALIFICATIONS

  • Bachelor's Degree from an accredited college or university.
  • Minimum of 2 years project management or related job experience.
  • Experience in a healthcare setting and/or as a Community Health Worker (CHW) preferred.
  • Bilingual in English and Spanish.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal, time management and organizational skills.
  • Ability to work both independently and as a member of an integrated team.
  • Proficient in Microsoft Office products.
