Minimum Qualifications:
  • Bachelor's degree required in a related filed.
  • Valid NJ Driver's License, Registration and Insurance required.
  • Minimum 5 years' PACE experience in a provider-facing role or community based organization.
  • Minimum 15 years' experience working with New Jersey Medicaid and/or commercial insurers.
  • Experience with the delivery of process improvement is required.
  • Strong interpersonal and communication and written skills required.
  • Ability to apply independent judgment and decision making required.
  • Proficiency in Microsoft Office Applications required.
  • Majority of the job is traveling to meet potential participants and their families, community groups, and referral sources for census growth.

POSITION SUMMARY

The LIFE Connection Outreach and Enrollment Manager is a high functioning community liaison role with account management and community sales responsibilities. This position is responsible for establishing relationships with referral sources and managing incoming referrals to ensure conversion from referral to enrollment. LIFE Connection's key contacts include community based organizations, primary care providers, specialists, sub-acute and specialty facilities and other value based provider partnerships. Performs initial intake assessment including medical history, medication review and bio-psychosocial assessment. In conjunction with the Director of Enrollment, the Manager is accountable for implementing the organization's marketing and enrollment plan and hitting key growth targets monthly by working in collaboration with the LIFE Connection team, prospective enrollees and their families, and community services/health care providers.

The Outreach and Enrollment Manager is LIFE Connection's primary contact with the appropriate county's board of social services for the purposes of facilitating enrollment of participants in the LIFE Connection. This position requires strong decision making skills, local market knowledge and visibility, an understanding and value proposition of LIFE Connection services, and a thorough understanding of the complex Medicaid application process.

This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.

QUALIFICATIONS

EDUCATION: Bachelor's degree required in a related field.

LICENSE/CERTIFICATION: Valid Driver's license required.

EXPERIENCE: Minimum 5 years' PACE experience in a provider-facing role or community based organization. Experience with the delivery of process improvement is required. Strong interpersonal and communication and written skills required. Ability to apply independent judgment and decision making required. Proficiency in Microsoft Office Applications required. Majority of the job is traveling to meet health care providers required.

WORK ENVIRONMENT

This position requires repetitive motion, fingering, hearing, pushing, reaching, reading, calculating, seeing, speaking, standing and walking a majority of the time. Works with such equipment as computer terminal, fax machine copier and telephone. Occasional driving also required.
