TITLE: Outreach Worker/Student Advocate

DEPARTMENT: Youth and Community Development Services

LOCATION: Community Schools - PS 328, East New York , Brooklyn

REPORTS TO: Community School Director

SALARY: $36,500 (annually) with excellent benefits including medical, dental, vision, prescription, life insurance, 401K plan and 24 vacation days per year.

SCHEDULE: Monday-Friday; some Saturdays included. Schedule may vary according to program needs.

JOB SUMMARY: The Outreach Worker/Student Advocate will work closely with families and Community School staff to support parents skills, parent-child relationships, family relationships, and parent-school communication. The Outreach Worker/Student Advocate is responsible for initiating and maintaining a regular and sustainable supportive relationship with high-risk families via home-based services. The Outreach Worker/Student Advocate will ensure optimal academic, social, and emotional development of both parents and students. The Outreach Worker/Student Advocate will work closely with the Community School Team (Director, Social Worker, Case Manager, and Tutor) to provide resources that will enhance the Community School.

EDUCATION, EXPERIENCE AND SKILL REQUIREMENT:

  • High School Diploma or General Equivalency Diploma (GED) required; College Degree preferred.
  • Two (2) years or more experience in the field of youth development.
  • Experience in Family outreach and Home Visits required.
  • Candidate should have excellent written and verbal communication skills.
  • Excellent collaborative, organization and time management skills.
  • Facility to motivate others toward achieving goals
  • Ability to develop alliances with high risk and difficult to engage youth and families.
  • Knowledge of community resources and counseling practices with high risk populations.
  • If hired, Grand Street requires all potential employees to provide proof of full COVID-19 vaccination.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Work with Community School team and community providers to conduct community outreach and promote program.
  • Collaborate with hospitals, clinics and community service provides to engage families in community events.
  • Create and maintain thorough written record of assessments, outreach efforts and all required documentation.
  • Work collaboratively with community resources to establish timely and effective referral processes.
  • Maintain ongoing open communication with school principal, parents, and school-based staff.
  • Through home visits, engage families and develop relationships.
  • Introduce and lead parent workshops that promote academic, social, and emotional development of both parents and students.
  • Partner with families to create family goals/plans that enhance parent and child well-being.
  • Maintain written records of home visits, family goals, and all required documentation.
  • Work collaboratively with community resources and advocate for families when necessary.
  • Refer participants to social worker for counseling to support with crisis intervention and safety planning.
  • Work closely with Social Worker, Tutor, and Case Manager to support targeted student needs.
  • Attend all required department/agency meetings and/or trainings (including attend attendance meetings to present attendance data).
  • Perform other duties as assigned by the Community School Resource Coordinator.
  • Working side by side with the schools Parent Coordinator to assist with daily phone calls, and daily attendance collection.
  • Will work in the main office to support office secretaries to compare attendance data and information.
  • Responsible for maintaining a mentee caseload of 15-20 Students.
  • Build positive relationships.
  • Check-in with students weekly individually or in a group of five (5).
  • Maintain presence in classrooms to observe student behaviors and areas of needed support.
  • Provide academic and behavioral support where needed.
