Position Purpose: We are seeking friendly, engaging, and outgoing team members to provide feet on the street outreach to our members. Your primary duties will include welcoming new members and securing completion of a health needs screening. To excel in this role, you will be confident, outgoing, persistent, and have excellent communication and organizational skills.

Requirements:

Responsibilities:

  • Conduct outreach to new members within 90 days of enrollment (either in provider offices or by going door to door)
  • Secure completed Health Needs Screening
  • Verify member information and eligibility
  • Manage workload according to deadlines
  • Clearly communicate Anthem programs available to new members
  • Enroll members into additional programs, and respond to inquires as they arise


Experience/Skills:

  • Minimum of two (2) years of experience in either volunteer or paid position working in community settings with at risk populations providing coordination of services is preferred
  • Proficient with Microsoft Office, including Outlook, Word and Excel
  • Sensitivity to and experience working within different cultures
  • Excelent interpersonal skills
  • Ability to work independently and as part of a team environment
  • Ability to identify problems and opportunities and communicate findings to management
  • Developing knowledge of local, state & federal healthcare laws and regulations & all company policies regarding case management practices
  • Demonstrate compassion, support and collaboration with members and families
  • Comfort with asking pertinent questions
  • Awareness of community & state support resources
  • Ability to keep composure and professionalism during times of high emotional stress
  • Ability to maintain confidentiality and act in the company's best interest


Position Requirements:

  • Proof of COVID-19 vaccination required
  • Mobile Worker: This is a mobile position, meaning that regular travel to different work locations is essential
  • Reside in the same territory as assigned to work in; exceptions may be considered, due to business need
  • May be required to travel greater than 50% of time to perform work duties. A valid driver's license, car, and insurance are necessary for work related travel
  • Flexible hours, including possible evenings and/or weekends as needed to serve the needs of our members
