Vail Place is looking for a full-time Adult Mental Health Case Managers to provide services in our Targeted Case Management program. Our growing program assists individuals with a severe and persistent mental illness (SPMI) to get the supports needed to accomplish goals and find recovery from mental illness. Case Managers are responsible for providing assistance to people with severe and persistent mental illness, both voluntary and involuntary referrals, who are in need of support and services to maintain safety and stability in the community. This vital program area addresses their many needs to ensure continuity of care, referrals to needed resources, coordination and monitoring of services, discharge planning, housing support, and intensive goal planning.

Work Environment: Keeping us Safe
In response to the COVID19 Pandemic, Vail Place has worked diligently to establish strong health and safety protocols for our community. This position is typically a face-to-face, in-person role. Currently many job duties can be fulfilled by phone or video-assisted technology, but some situations will require in-person meetings. Any interactions that require in-person contact will involve the use of social distancing and personal protective equipment. Vail Place grounds are also tobacco free to promote health and wellness for the individuals we serve and for our employees. Applicants will be required to perform in-person work in the Twin Cities on a regular basis.

Salary and Benefits
Starting at $46,000 - $49,500 Depending on Qualifications . Full Benefits Package including medical, dental, vision, life, AD&D, long term disability, matched 401K. Paid time off in Year 1 includes generous vacation & sick/safe time, plus 3 personal days and 13 paid holidays.
