POSITION SUMMARY
The Lead CHHA position is responsible for participating in the coordination of program CHHA's by supporting the HomeCare Coordinator. The Lead CHHA is responsible for ensuring CHHA schedule for Homecare and Center is up to date, accurate and available to all staff. Position will ensure the Day Center is attended by a CHHA at all times by providing monitoring and oversite. This position is also required to perform all CHHA duties as needed, including patient care, activities of daily living, restorative and supportive care, and recreational needs. The Lead CHHA ensures that the patient care is in accordance with the plan of care assigned by the primary nurse, and complies with other established standards. The Lead CHHA is also responsible for ensuring there are accurate Care Plans for each participant in multiple venues of care. The Lead CHHA is a required member of the LIFE Connection IDT and is expected to attend daily meetings. This position works with nursing leadership to order supplies for participants. This position is responsible for providing guidance to the program CHHA's for educational needs, skills training, CHHA preceptorship, HR requirements, and state yearly educational requirements.
This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.
QUALIFICATIONS
EDUCATION: High School diploma or equivalent required. Successful completion of a home health aide training program meeting national standards required.
LICENSE/CERTIFICATION: Home Health Aide certification in the State of New Jersey required. Valid NJ driver's license, valid registration and insurance required. Safe driving record also required.
EXPERIENCE: Minimum 2 years working with the frail or elderly. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.
PERFORMANCE EXPECTATIONS
Demonstrates the technical competencies as established on the Assessment and Evaluation Tool.
WORK ENVIRONMENT
This positon must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.
This position also requires corrected vision and hearing to normal range. Incumbents must be able to communicate with all staff, vendors, and clients. Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment. This position also requires lifting/carrying up to 30 pounds. Using appropriate body mechanics and equipment, with reasonable accommodation if needed.
