I. GENERAL STATEMENT OF RESPONSIBILITIES:

To provide needed and appropriate direct services to Independent Living Plus (ILP) youth and to coordinate the provision of collateral services as required by State of Michigan Laws, Services Manual Items, Child Welfare Licensing Requirements and applicable Service Standards.

II. QUALIFICATIONS:

A. Education: High School Diploma (or equivalent, as defined by Child Welfare Licensing), Bachelor Degree preferred.

B. Experience: One year of child welfare experience.

C. Skills:

  • Transportation: Reliable and ongoing as required to make home visits and provide transportation to youth. Valid, unrestricted Drivers License. Good driving record
  • State of Michigan Licensing Requirements: Satisfactory Department of Health & Human Services (DHHS) Central Registry Clearance and State Police Record Check. Negative TB test
  • Satisfactory physical exam within one year of hire
  • Provide proper care to ILP youth according to regulatory, funding and service standards
  • Reports any noncompliance or deficit in proper care or unmet treatment needs to the immediate Supervisor, and the Program Director as per program procedures
  • Works cooperatively with ILP Supervisor/Case Manager to develop treatment goals and standards of proper care
  • Provides supervision to Independent Living Plus (ILP) youth in the home and in the community to establish appropriate social patterns and behaviors to help client fulfill needs for independence; this will include consistent work habits, budgeting, maintenance of living environment
  • Trains new staff, as assigned
  • Conducts in-house orientation, as assigned

D. Knowledge:

  • Secures or advocates for an appropriate educational placement for each youth within five days of placement and on an ongoing basis by participating in IEPC, school conferences, and meeting with appropriate school personnel
  • Makes appropriate referrals and secures needed services from collateral agencies and community services, in cooperation with ILP Supervisor/Case Manager
  • Reads and stays familiar with all current Services Standards applicable to the Independent Living Plus Program, i.e., Department of Health & Human Services (DHHS) Manual Items, Program Manual and Master Contract
  • Prepare for, attends on time, and actively participates in all required meetings, such as staff meetings, quarterly case conferences
  • Actively participates as a team member in maintaining the Independent Living Plus Contract
  • Displays professional courtesy by being on time for visits, appointments, meetings, and therapy, canceling or rescheduling ahead of time if possible, and arranging for coverage during absence or illness
  • Completes all facets of documentation pertaining to clients such as bi-weekly record of contacts (including activities engaged in, observation of behaviors); house log, Unusual Incident Reports (UIRs)
  • Serve as client advocate
  • Attends and participates in staff meetings
  • Attends and participates in at least 25 hours of training and/or all required or assigned training and conferences
  • Complete all agency required training within the specified time frame

III. DIRECTLY SUPERVISES: Not applicable

IV. GENERAL RESPONSIBILITIES:

  • Completes all tasks and duties as may be assigned from time to time by Spectrum administrative staff
  • Complies with all Spectrum Personnel Policies and Procedures
  • Complies with all other agency program policies and procedures
  • Works cooperatively with other agency staff, serving on committees, completing special projects and participating in agency decision-making as needed
  • Completes and submits required documentation such as time sheets/ADP, mileage reimbursement requests, petty cash, daily log, and bi-weeklies on time
  • Maintains the quality of the home environment by taking initiative to see that household cleaning, minor repairs and maintenance of home property and grounds are tended to and meet safety, sanitation and acceptable living standards
  • Report promptly maintenance and repair needs at the house to the ILP supervisor
  • Report all client and staff injuries immediately
  • Know and follow emergency home procedure for fire, tornadoes, accidents, and other serious incidents affecting a clients welfare
  • Be able to work under stress, maintaining a positive approach to position and agency

V. SPECIFIC DUTIES:

  • Establish a relationship with the youth upon placement to assist in integrating the youth into placement. This includes establishing a routine for the youth consistent with the treatment plan
  • Assists client with goals as defined in the Independent Living Contract
  • Assists and supervises client with problem solving, task completion, chores, homework, and daily routines
  • Acts as a positive role model for clients
  • Keeps supervisor fully informed of case progress and case status orally and in writing as required (completes daily/weekly forms), meets with supervisor individually, in staff meetings, and in other meetings as may be required
  • Provide enriching experiences for the client through outside activities while working to improve the youths functioning in the community
  • Travels to various locations (homes, school, employment, and community resources) to facilitate the accomplishment of the treatment plan.
  • Participates in a system of 24-hour staff availability to consumers in crisis. Responds within fifteen minutes when contacted
  • Protects confidentiality appropriately, sharing information for the benefit of the client, and obtains releases of information as necessary
  • Immediately reports any incidents of actual or suspected client abuse to Independent Living Plus (ILP) Supervisor and/or Program Director
