SUMMARY: Under the direction of the Clinical Supervisor the Discharge Planner is responsible for coordinating continued care and establishing community-based resources to be utilizedfor clientswho arecompleting residential care at John Brooks Recovery Center
JOB RESPONSIBILITIES:Responsible for utilizing the SMART clinical documentation program to maintain clinical recordsResponsible for facilitating and solidifying initial discharge plan with clients within first two weeks of admissionFacilitate the transitioning of clients in DAS funded treatment slots to SAI slot to provide funding for aftercare or treatment continuum placementsParticipate in residential unit multidisciplinary meetingsMust be knowledgeable of DAS regulations regarding discharges and discharge planningMust be knowledgeable in the core function of referral and maintain a list of referral sourcesConduct presentations on various referral sources for staff membersPrepare and maintain a referral/resource guide for residential unitsRequired to meet with individual counselors to coordinate transition for clients who are being dischargedAssist counselors with determining clients clinically driven lengths of stayCase manages and assures an integrated approach to meeting the discharge needs of male residential clientsPerform other related duties as assigned by immediate supervisor and other management as directedREQUIRED KNOWLEDGE, SKILLS AND ABILITIES:Demonstrate interviewing and communication skills in a caring and respectable mannerAbility to collect, organize and maintain client folders within designated timeframeDemonstrate knowledge of state regulations regarding client chartsDemonstrate ability to utilize JBRC computer system to input clientinformationKnowledgeable in addiction terminologySUPERVISION RESPONSIBILITIES:None
QUALIFICATIONSBA/BS degree required. One years experience in the collection of data in an interview setting.
