SUMMARY: Under the direction of the Director of Clinical Services and/or Clinical Supervisor, the Counselor (MAT Part Time Evening) is responsible for the professional delivery, documentation and reporting of all treatment services to include by not limited to; counseling (individual, group, family and crisis) and comprehensive case management to clients.

JOB REQUIREMENTS:

Each substance abuse counselor shall be responsible for the following:

1.? Assessing the counseling needs of the clients;

2.? Assessing clients using the ASI or using other standardized assessment tool and for adolescents using the CASI or using other evidence-based validated assessment tool and diagnosing clients for substance disorders using the DSM-IV-TR;

3.? Determining the appropriate level of care based on ASAM PPC-2R;

4.? Obtaining previous records that are relevant to the current treatment episode;

5.? Collaborating with the client in order to develop a written treatment plan that is client-centered and recovery oriented and includes goals and measurable objectives.

a.? The director of substance abuse counseling shall develop, monitor and provide clients and the treatment team with written schedules that will be used to update each client treatment plan(s) in order to ensure that each client's treatment needs are addressed;

6.? Providing the substance abuse counseling services specified in the client treatment plan;

7.? Reviewing clients throughout the treatment episode according to ASAM PPC-2R, to determine the need for continued services or discharge/transfer;

8.? Reviewing and, where necessary, revising the substance abuse counseling portion of the client treatment plan to address emerging problems;

9.? Developing the client discharge/transfer plans to ensure movement to the appropriate levels of care;

10.? Contacting referral sources, providing case consultation and coordination with referral sources (for example: mental health treatment providers, criminal justice agencies, schools, employers, the Division of Youth and Family Services);

11.? Participating as a member of a multidisciplinary team for assigned clients;

12.? Providing active case consultation; and

13.? Documenting all counseling and education services, assessments, reassessments, referrals and follow-up in the client's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records.

JOB RESPONSIBILITIES:

1.? Maintains a caseload consistent with state, federal and JBRC standards and responsible for completing the tasks associated with maintaining a case

2.? Responsible for maintaining contact with referral sources as required.? i.e. DOC, DCI, Parole/probation.

3.? Responsible for developing a treatment plan which identifies and ranks problems.

4.? Responsible for identifying the treatment methods and resources to be utilized based on individual client needs.

5.? Provides group, individual, family counseling in accordance with Attachment C and present clients on caseload to Treatment Team review every 30 days.

6.? Maintains a written and electronic chart in SMART on each client to include all services rendered.

7.? Responsible for keeping supervisor informed of any significant treatment issues that occur with clients.

8.? Participate in all required meetings and training.

9.? Responsible for accurately preparing and submitting all required reports for co-signature of supervisor according to schedule of the Administrative Assistant to be forwarded to appropriate agencies and funding sources.

10.? Maintains confidentiality for all client information in accordance with state and federal regulations.

11.? Employee is expected to come to work on time and schedule time off.

12.? Employee is expected to? exhibit willingness and cooperativeness with co-workers and supervisor, ability to accept constructive criticism

13.? Performs other related duties as assigned by the Director of Clinical Services and/or Clinical Supervisor and other management as required.

14.? When necessary, CADC Interns, working towards LCADC, must have work signed off by LCADC.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

1.? Ability to gather relevant history from client including but not limited to alcohol and other drug abuse using appropriate interview techniques.

2.? Ability to search out new task and expand abilities professionally and personally.

3.? Capable of explaining to the client the rationale for the use of assessment techniques in order to facilitate understanding.

4.? Demonstrates the ability to develop a diagnostic evaluation of the client's substance abuse and any coexisting conditions based on the results of all assessments in order to provide an integrated approach to treatment planning based on the client's strengths, weaknesses, and identified problems and needs.

5.? Capable of identifying and ranking problems based on individual client needs in the written treatment plan.

6.? Ability to formulate agreed upon immediate long-term goals using behavioral terms in the written treatment plan.

7.? Capable of identifying the treatment methods and resources to be utilized as appropriate for the individual client.

8.? Ability to apply techniques to assist the client, group, and family in examining the client's behavior, attitudes and feelings if appropriate in the treatment setting.

9.? Demonstrate the ability to interact with the client in an appropriate therapeutic manner and with staff and visitors in a professional manner..

10.? Capable of coordinating services for client care.

11.? Ability to recognize the elements of the client crisis and implement an immediate course of action appropriate to the crisis, while enhancing the overall treatment by utilizing crisis events.

12.? Capable of presenting relevant alcohol and other drug use/abuse information to the client through formal and informal processes.

13.? Ability to match client needs and/or problems to appropriate resources.

14.? Capable of preparing reports and relevant records integrating available information to facilitate the continuum of care.? Ability to chart pertinent ongoing information pertaining to the client, while utilizing relevant information from written documents for client care, meeting company standards, including neatness.

15.? Capable of multi-tasking and detail orientated.

16.? Ability to establish and maintain a positive and cooperative working relationship with all employees.

17.? Ability to be respectful of diverse cultures, ethnic, and religious backgrounds.

18.? Establishes and maintains a positive and cooperative working relationship with outside agencies.

19.? Ability to deliver service in a way that is responsive, respectful, caring and helpful while maintaining confidentiality.

20.? Ability to demonstrate responsiveness and flexibility as client's needs and priorities shift.

QUALIFICATIONS:

BA/BS Degree in social work or related field

Minimum Credential is a CADC, LCADC, LSW or LAC
