The Community Health Worker (CHW) participates as a member of the care team, working within the community setting, providing outreach to patients with complex medical and social needs. The CHW acts as a resource to assist patients attributed to Value Based Programs in navigating the health system; and serves to facilitate linkage to community resources under the direction of the providers and RN care manager. The CHW will be responsible for all care gap outreach; additional responsibilities such as Social Determinant of Health Screenings, ED follow up and connecting patients to community resources such as housing, food, substance abuse programs, etc.

We believe that Community-Health Workers (CHWs) have the skills and experience to understand what low-income patients are going through and help them get through difficult times. CHWs are people who come from the communities they serve. CHWs act as caring neighbors, to help patients address the social, medical and community problems that lead to poor health. The goal is to help patients deal with the'real life' issues that keep them from staying healthy, help with the tasks of getting medical care, and work on health goals by doing things like planning healthy meals or finding time to exercise.

The CHW collaborates with the multidisciplinary team in developing and implementing the care plan, and participates in the practice weekly or daily huddles. This position engages patients by ways of telephonic coaching, face to face visits at provider appointments, ED, or in the community. The CHW facilitates adherence to treatment plan by early identification and resolution of care gaps. This position provides appointment scheduling and other clerical duties as needed and assigned by program manager or designee.

EDUCATION: High School Diploma required. Associates degree preferred. LICENSE/CERTIFICATION: BLS certification required within 6 months from date of hire or transfer. Valid NJ Driver's License, Insurance and Registration required.
EXPERIENCE: Long-time resident of community with good knowledge of the resources available within Atlantic City and Atlantic County, NJ.
Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.

Demonstrates the competencies as listed on the Assessment and Evaluation Tool for this position.
Potential for exposure to the hazards of the hospital environment, such as exposure to infectious disease, blood-borne pathogens, hazardous waste, and potential injury. This position requires considerable standing, reaching and walking. The ability to lift up to 50 lbs., is required.
Community position travel required, personal vehicle with mileage reimbursement.
