Position Objective:

The Certified Community Health Worker is responsible for serving as the initial and main point of contact between the company and the member. Supports member access to care through face-to-face visits to complete health screening and needs assessments. Refers member to community resources, processes reports, and distributes collateral materials.

Position Responsibilities but are not limited to:

  • Coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs
  • Will provide telephone and/or on onsite Recovery Support
  • Makes referrals to the Case Management Services
  • Assesses member compliance with medical treatment plans via telephone or through on-site visits
  • Identifies barriers to plan compliance and coordinates resolutions while ensuring alignment with internal company team
  • Provide assistance accessing various community resources (education, housing, transportation, recovery supports, etc.)
  • Recommends treatment plan modifications and determines need for additional services, in conjunction with case management and provider
  • Coordinates identification of and referral to local, state or federally funded programs
  • Participates in cross-functional teams on projects, initiatives, and process improvement activities
  • Maintains the confidentiality of all sensitive information. Accountable to understand role and how it affects utilization management benchmarks and quality outcomes

Requirements:

Knowledge/Experience:

  • High School Diploma or equivalent GED.
  • BA/BS in social work and/or directly related field is preferred.
  • 1-3 years of experience in health services or behavioral health field.
  • Peer support services role (or any combination of education/experience) is required
  • Community Health Worker Certification/Recovery Specialist Certification preferred
  • Bilingual candidates preferred
  • Working knowledge of community based populations, medical conditions and/or disabilities
  • Must have strong interpersonal and communication skills
  • Must be able to communicate effectively with people from a diverse socio-economic levels and backgrounds
  • Must have good organizational skills
  • Must be able to work independently and effectively with others
  • Ability to handle protected health and personal information in accordance with HIPAA requirements.
  • Certified nurse assistant or certified medical assistant and/or BS/BA degree in a related field preferred.
  • Ability to obtain a valid Indiana driver's license and access to reliable transportation.

Physical Demands/Work Environment:

  • Proof of COVID-19 vaccination required
  • Ability to continuously stand or walk. Ability to bend and climb stairs with the potential for frequent lifting. Ability to lift up to 25 pounds, occasionally.
  • This position requires individuals to work from home or an established work site. Must have a dedicated office space in home environment.
  • This position requires frequent driving - applicants must provide proof of an active driver's license, insurance, and must have reliable transportation.
  • This position may require employee to conduct home visits. Requires individuals to obtain and exchange information and assist the member with resolving simple to complex issues related to accessing their health care benefits. This work requires frequent communication - must have the ability to receive information and express self in an appropriate proferssional manner. Employee must exercise problem solving skills and caution when entering homes to ensure safe environment.
