POSITION SUMMARY

The Case Management Support Staff (CMSS) role provides clerical support to ensure patients achieve a smooth and timely transition from an acute care facility or in the outpatient setting to help prevent re-admissions. This position assists with setting up outpatient services or providing resources to help the patient maintain optimal care. This position performs follow-up tasks identified during the hospital assessment for management of patients across the healthcare continuum or when the patient is in the continuum and needs additional resource support. This position also functions at the direction of a Registered Nurse or Social Worker Case Manager.

This position works to coordinate transitions into or out of care setting in order to obtain appropriate services and benefits. This may include faxing information, performing referrals or tasking other departments or consultants. The Support Staff also may arrange authorization and transportation, assure transfer documentation completion, arrange DME, coordinate Home Health Care an confirm arrangements and appointments at the direction of the Case Manager.

This position also performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. The Case Management Staff also provides all customers of AtlantiCare with an excellent service experience by consistently demonstrating our core values every day.

This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.

QUALIFICATIONS

EDUCATION: High School Diploma required. Associate's Degree preferred.

LICENSE/CERTIFICATION:

EXPERIENCE: Minimum 3 years secretarial experience required. Medical Terminology required. Strong effective communication skills, human relations skills, strong organizational skills, time management skills, and flexibility in responding to multiple demands is required. 3 years of Patient Care Technician, Medical Assistant, or Administrative Associate on a patient care unit preferred. Previous insurance experience preferred.

PRFORMANCE EXPECTATIONS

Consistently demonstrates the technical and behavioral competencies as established on the Assessment and Evaluation Tool.

WORK ENVIRONMENT

Office environment with limited exposure to infectious disease, hazardous substances and potential injury. This position requires desk/computer work a majority of the time.

This position reports to department leadership.
