Purpose:

Provide therapy, community support, case management, and consultation services as needed to youth, families, and adults with mental illnesses and to their support systems. Services consist of family-oriented and trauma-informed therapeutic interventions that facilitate healing, skill-building, identification and use of natural supports, and use of community resources. Work collaboratively as part of an outpatient team. Articulate C4's mission and vision.

Work Conditions:

Services will be provided mostly at the agency, with occasional community-based work necessary. Some evenings required.

Minimum Qualifications:

  • Master's degree in social work, psychology, counseling, or a related field is required
  • One or more years of experience working with children and adults with mental illnesses is required. This can include internship experiences.
  • Excellent interpersonal, verbal, and written communication skills
  • Spanish fluency

Competencies (Minimum Skills, Knowledge, and Experience):

  • Ability to be respectful of the diverse cultures of the people served and to provide culturally appropriate, anti-racist, competent, and individualized support according to each client's age, race, gender identity, sexual orientation, ethnicity, religious beliefs, and culture
  • Experience and high level of interest in working with youth, families, and adults
  • Knowledge and experience with integrating mental health and physical healthcare
  • Ability to use trauma-informed and evidence-based interventions
  • Understanding and use of personal and professional boundaries
  • Excellent time management skills, as some services will be provided at various locations in the community
  • Excellent organizational skills as completing documentation in a timely manner is required
  • Teamwork and collaboration skills
  • Conflict-resolution and diplomacy skills
  • Problem-solving skills
  • Ability to demonstrate flexibility to meet clients' and agency's needs

Duties and Responsibilities:

  • Provide comprehensive mental health assessments, treatment planning, therapeutic services, case management, and treatment evaluation for youth and adults with a mental illness in person and via telehealth
  • Facilitate treatment interventions to maintain or enhance the client's capacity for healing and a return to a person-centered baseline of functioning
  • Provide services in the client's home, or in the community near the client's home, to overcome obstacles to the client's access to services, and to support the client in using community resources as fully as possible
  • Interview and evaluate clients
  • Develop treatment plans collaboratively with the client by defining goals, specific outcomes, and time frames for treatment
  • Complete all necessary documentation (Mental Health Assessments, Treatment Plans, Progress Notes, etc.) in a complete and timely fashion, meeting agency standards/requirements
  • Work collaboratively with the Clinical Supervisor and other members of the Children Outpatient team, as well as other community service providers, to coordinate and integrate all aspects of care
  • Adhere to confidentiality practices and procedures according to agency policies
  • Always maintain compliance with agency policies and procedures
  • Meet direct service hour expectations for the benefit of clients
  • Other duties as assigned by supervisor
